RELEASE AND WAIVER FORM

If you are under 18 years of age at the time of completing your application, you must have a parent or legal
guardian read and understand the preceding release and waiver, and agree to the terms of the release and waiver.

1.  Waiver and Release. I hereby release Surrey 2016 WBSC XV Women’s World Softball Championship and the Canadian Open Fastpitch International Championship, the organizers, and sponsors of this event, from all liability, claims, and demands of
whatever kind or nature, either in law or in equity, which arise or may hereafter, arise from my participation with Surrey 2016 WBSC XV Women’s World Softball Championship and/or Canadian Open Fastpitch International Championship.

2.  Assumption of Risk. I understand and acknowledge that my time with Surrey 2016 WBSC XV Women’s World Softball Championship and/or Canadian Open Fastpitch International Championship could include activities that may involve certain elements of risk or the chance of injury. I hereby expressly and specifically assume the risk of injury or harm and release Surrey 2016 WBSC XV Women’s World Softball Championship and/or Canadian Open Fastpitch International Championship from all liability for injury, illness, death, or property damage resulting from the activities of my time as a volunteer with Surrey 2016 WBSC XV Women’s World Softball Championship and/or Canadian Open Fastpitch International Championship. I also understand that Surrey 2016 WBSC XV Women’s World Softball Championship and/or Canadian Open Fastpitch International Championship does not assume any responsibility for or obligation to provide financial assistance or other assistance, including but not limited to medical, health, or disability insurance, in the event of injury, illness, death, or property damage. You are expected and encouraged at all times to carry your insurance card while volunteering.

3.  Photographic Release. I hereby consent to the reproduction, publication and use of photographic images, video and audio
recordings taken of me / my child at Surrey 2016 WBSC XV Women’s World Softball Championship and/or the Canadian Open Fastpitch International Championship. Such content will not be used for resale purposes, but may be featured in promotional or highlight materials such as television, radio, film, newspapers, magazines, online and other media, and in any form, for the purpose of advertising or communicating the purposes and activities of Surrey 2016 WSBC XV Women’s World Softball Championship and/or the Canadian Open Fastpitch International Championship.

I understand that I will neither own or be paid for the Recordings. I hereby release and discharge any right, title or interest that
may have in the Recordings or in any remuneration for using the image / video / audio recordings.

4.  Code of Conduct and Policies. I agree to abide by all applicable policies, codes and rules, as may be amended from time to
time, and will follow all instructions of the Surrey 2016 WBSC XV Women’s World Softball Championship and/or the Canadian Open Fastpitch International Championship Organizing Committee in carrying out the volunteer activities. I understand failure to follow Surrey 2016 WBSC XV Women’s World Softball Championship and/or Canadian Open Fastpitch International Championship policies may result in termination of volunteer activities.

The Organizing Committee will ensure that the personal information you provide to us is protected against loss or misuse by such security safeguards, as it is reasonable in the circumstances to take.

**Please note that all applicants will undergo a screening process and that a Batgirl role will not automatically be offered.
